Appalachian State University Academy at Middle Fork Surveys of Students
I. Introduction
Appalachian State University Academy at Middle Fork (the “Academy”) shall ensure that all notification and other requirements of state law and the Protection of Pupil Rights Amendment are met, including all legal requirements regarding the surveying of students.
II. Policy Scope
This policy shall apply to all faculty, teachers, staff, and students at the Academy.
III. Definitions
3.1 Protected Topic Survey:Any survey that asks or includes any of the following topics:
political affiliations or beliefs of the student or the student’s parent.
mental or psychological problems of the student or the student’s family.
sex behavior or attitudes.
illegal, anti-social, self-incriminating, or demeaning behavior.
critical appraisals of other individuals with whom respondents have close family relationships.
legally recognized privileged or analogous relationships, such as those of lawyers, physicians, and ministers.
religious practices, affiliations, or beliefs of the student or the student’s parent; or
income (other than that required by law to determine eligibility for participation in a program or for receiving financial assistance under such program).
For purposes of this policy, a “protected topic survey” is any survey, analysis, or evaluation that reveals information concerning any of the protected topics.
