Admission Guidelines & Eligibility

Our school aims to support students at all stages of their learning journey, however, lab school legislation requires that at least 80% of our enrollment consist of students who need additional academic support to reach grade level expectations. 

Please note that acceptance into our school is subject to one or more of the following factors:

  1. Grade Level Availability: Enrollment is dependent on the availability of spaces within the requested grade level. If the grade level is at full capacity, the student will be placed on a waiting list.
  2. Residential School Assignment: The student's eligibility for enrollment is influenced by their assigned residential school in WS/FCS and that school’s performance designation.  Students zoned to low-performing schools have priority enrollment.
  3. Academic Progress: Student's current academic progress in relation to meeting grade level expectations will be used to determine eligibility.

  • Kindergarten students are required to complete a kindergarten screening. 
  • 1st-5th grade students may be asked to provide one or more of the following documents:
    • Report Card
    • NC State Testing Results

If you have questions about eligibility, please contact us. Eligible students are accepted in the order in which the applications are received and by grade level and school capacity. Child must be at least five years of age on or before August 31 of that school year.

Apply for Enrollment with Online Registration in Infinite Campus (Link below)

 Step 1 – Enrollment Checklist items

Gather the following items before beginning the Online Registration so that you can upload the documents when submitting the enrollment form.  If you do not have these documents at this time, you will be able to provide them later.  Please note that all documents are necessary to complete the enrollment process.

  1. Your child’s Birth Certificate (state certified copy)

  2. Your child’s Immunization Records.

  3. NC Health Assessment completed by the medical provider

  4. A government-issued photo ID of the parent or legal guardian.

  5. Proof of residency in Forsyth County, which can be a current water, gas, or electric bill in the name of the parent or legal guardian, that is no more than 30 days old. For other acceptable documents, see here.

If applicable: 

         6. Most current report card and / or NC State Testing results

        7. EC paperwork such as IEP or 504 plans

        8. Court documents such as custody orders.

        9. Doctor’s orders such as prescriptions

Step 2 – Complete the Online Registration form to enroll at the App State Academy at Middle Fork.

Step 3 – Once the enrollment form has been reviewed, you will receive an email informing you if the application is missing data and needs more information. Or if it is complete, you will receive a call to schedule an appointment to meet with the principal.

Join us for a family STEAM Night where you can meet other families, learn more about the school, and participate in fun, hands-on science activities.
